Care-giver Support Program
For the primary care-giver of an aging or disabled relative, this can be a rewarding, yet consuming job. CareWise can help relieve the strain of care-giving by becoming a part of the support system.

Independent Living Program
For the elder who can continue to live at home but needs increasing assistance with life's details, CareWise will recommend, arrange for and supervise care providers that come into the home.

An in-home assessment of the elderly person's physical and emotional needs.

Coordination and monitoring of medical care and in-home care, including arrangement of support services.

Safe-home assessment recommending any modifications to reduce risk for falls.

Encourage quality family and social involvement which may include participation in senior center programs, support groups, adult day programs, holiday celebrations, etc.

Periodic written status reports to the family.
Alternate Living Placement Program
For the elder who cannot care for themselves at home anymore but do not meet the medical requirements for admission to a nursing home.

Nursing Home Placement Program
